We see patients from
8:00am to 6:00pm, Monday through Friday. Inquire about
evening and/or Saturday appointments.
First Visit
New Patients:
PRINT OUT the
new patient package.
Take 20 minutes and complete the first 8 pages of the
package. Bring the rest of the package with you on your
first visit.
Payment & Insurance
Payment is required at the time of service.
$55 per treatment, $10
evaluation fee required upon first visit
Additional fees may apply for
extended treatment time
We provide a
Superbill for our insurance holders
